Our family medicine providers can refill prescriptions for you as needed.
  • OB-GYN services: Some of our family medicine providers offer OB-GYN services, including Pap tests, birth control and prenatal visits.
  • Seamless care: Our goal is to make your care seamless. Through our patient portal, you can send questions directly to your family medicine team. If you go into the hospital, Íøºì³Ô¹Ï hospitalists communicate with your family medicine providers. And when you leave, our health care managers check in with you to make sure you have everything you need, including follow-up visits with your family medicine providers.

Preventive Visits

A preventive visit (annual wellness visit or routine physician exam) does not include treatment of an acute illness or management of a chronic condition.

Annual Wellness VisitRoutine Physical Exam

Includes:

  • Review and update your personal prevention plan (including health screenings and vaccinations)
  • Assess your health risks
  • Review your current medications
  • Take your height, weight, blood pressure and other routine measurements

Does not include:

  • Treatment of an acute illness or management of a chronic condition

Includes:

  • Take your height, weight, bloodwork and vital signs
  • Touch and listen review and exam of body systems
  • Discuss any changes to your health and create personal health plan for the year ahead

Does not include:

  • Treatment or diagnosis for a specific illness, symptom, complaint or injury
  • Treatment of an acute illness or management of a chronic condition
