Is it time for a skilled nursing facility?

Has your hospital physician determined that the need for acute hospital care is winding down? If so, he or she may have recommended a skilled nursing facility (SNF) or skilled nursing care for physical rehabilitation in preparation for a safe return home.

What is a skilled nursing facility?

A skilled nursing facility or SNF provides a high level of medical care through licensed health professionals, including registered nurses (RNs) and physical, speech, and occupational therapists. These services may be needed for a short period of time for rehabilitation from an illness or injury, or they may be required more long term for those who need close medical attention more frequently.

Skilled nursing care may also be provided in a home setting for a duration of time. However, at-home care must be ordered by a doctor in order to be covered by Medicare, Medicaid, the VA or private health insurance.

Making a difference

You or your loved one may, of course, transfer to any facility of your choosing. However, choosing one of our preferred listed facilities will enable us to remain hands-on and involved.

  • Your 厙綻勛圖 primary care physician will be kept informed of your progress.
  • A 厙綻勛圖 Care Manager may be in touch with you.
  • Well discuss discharge instructions and medications with you, answer your questions, and coordinate necessary services.
  • A follow-up doctors office visit will be arranged within 48 hours of discharge from the SNF.

Your action plan...

  • Review the below SNF list and discuss with your physician and family members as needed.
  • Should you so desire, make a first choice and an alternate SNF.
  • Inform hospital social worker involved in discharge arrangements of your SNF choice as soon as possible.
  • Transfer to your chosen facility. (Subject to availability and Medicare or insurance.)
